A medical chronology template is a table that lists each documented visit in date order, with the provider, what the record states and a page citation for every entry. Use the columns below, download the CSV, and keep findings limited to what the source page actually says.
Each column has one job. Findings as stated quotes or closely tracks the record; Attorney notes holds gaps, questions and strategy so commentary never mixes with clinical content. The Bates/page cite column lets a reviewer open the exact page behind each line. For layout variations such as narrative or issue-based formats, see our guide to medical chronology formats.
Every name, facility and Bates number below is invented. The rows illustrate format only and describe no real patient.
| Date | Provider | Facility | Visit type | Findings as stated | Treatment | Bates/page cite | Attorney notes |
|---|---|---|---|---|---|---|---|
| 2025-03-04 | Example emergency clinician | Example Emergency Department | Emergency visit | Patient reports neck pain after rear-end collision; cervical paraspinal tenderness noted | Cervical X-ray ordered; cyclobenzaprine prescribed | FICT-000014 to 000017 | Confirm mechanism of injury matches police report. |
| 2025-03-11 | Example rehabilitation clinician | Example Rehabilitation Clinic | Initial evaluation | Reduced cervical range of motion; pain rated 6/10 by patient | Chiropractic adjustment; home exercise plan | FICT-000032 to 000035 | Note 7-day gap from ED visit. |
| 2025-04-02 | Example imaging clinician | Example Imaging Center | Imaging | MRI cervical spine: C5-C6 disc protrusion as described in radiology impression | None recorded | FICT-000058 | Request films if causation is disputed. |
| 2025-04-18 | Example follow-up clinician | Example Follow-up Clinic | Follow-up | Persistent neck pain radiating to right arm per patient report | Referral to physical therapy; follow up in 6 weeks | FICT-000071 to 000073 | Records for referred PT not yet received. |
The CSV opens in Excel, Google Sheets or Numbers. Delete the fictional rows before adding entries from your own file. To see a longer worked example with review flags, open our fictional sample chronology.
Start with a record inventory: which providers you requested, which responded and which pages arrived. Then add one row per encounter, citing the first and last page that support it. When two records disagree, enter both and flag the conflict in the notes rather than choosing one. The step-by-step process is covered in our medical chronology guide.
Manual chronologies tend to drift: citations go missing, paraphrases get stronger than the record, and records that were never received are not noted. Software-drafted chronologies fail differently. Dates and handwriting on scanned pages can be misread, separate visits merged, or an entry attached to the wrong provider. Either way, someone must check each line against its cited page before the chronology is relied on.
If you are weighing a spreadsheet against software for repeat matters, compare the time your team spends on drafting and checking with the costs on our per-case pricing page.
